During the final phase of the assessment, families are prepared for integration into the community and independent living. The support includes:
- Integration with community resources such as support groups, children centre and parent-child groups
- Signposting to appropriate support services where applicable
- Free aftercare support for up to 3 months

Other Services
- Supervised Contact for families within the assessment
- Free aftercare service for up to 3 months
- Outreach Support for families who have successfully completed assessments with a positive outcome.
- Step Down – We offer a step-down program where it is identified that this is required as part of the reintegration process.
